Thanks. I went a different by using acf extended to create a form. The form shortcode puts the form wherever I want, and allows me to send parameters to the form as well.

You could create your own shortcode, but a more powerful solution is to download Tangible: Loops & Logic (the successor to Custom Content Shortcode). Loops&Logic has replaced several plugins for me.
How about using the acf class attribute?

For anyone else following this, I couldn’t just add that CSS to a style sheet. I needed to create a snippet like this:

<style type="text/css">

add_action('acf/input/admin_head', 'my_acf_admin_head');
