Visitor input to fields

Hello,

I am looking for a way to let a visitor enter two numbers (or two words). When both numbers/words are entered the values need to populate two custom fields. Then a redirect to a page when i can use those two custom fields.

Can you help me with some code (html / php / gutenberg)?
