Taxonomy Image

I’ve created an image field where ‘Taxonomy Field’ is equal to ‘Region’ (custom post type).

when I add a new region, the field appears and allows me to add one. However when I go to edit an existing one the field doesn’t appear even if I’ve add it as new and then edit.

Any ideas why??
