Strange file field view

I have latest ACF 5.3.2.1 and WP and for some reason my file field looks odd:

i am not sure since when it looks like this. and no matter which layout i choose. it always overlaps

Do you have some type of plugin installed, or a filter that’s altering the image icon used for a PDF file?

The attached image is what I see with a default WP install and no other plugins.
