Problems using acf_form() twice on the same page

On one of my pages I need to call acf_form() twice. The first form being called works correctly, but on the second form any fields using JS are broken. If I remove the first form, the second one works correctly. Has anyone encountered this issue? I’m using ACF 4, not 5 for various reasons.

This is quite odd, what kind of errors are logged on the page?

I am not sure about the structure that you are after, have you considered using the second form with the ‘form’ parameter set to false.
