Make a single page for data.

I’ve used the advanced custom forms to make a form that allows the entry of multiple items that have a title, image and description. I was wondering if its possible to make a link for every item to their own page. Similarly to how wordpress works with a posts page and a single post.

make a custom post type which is public and you should be fine.
