issues with Repeater field

I have installed ACF and the repeater field onto a new website that I am building for a client, however it does not seem to work at all.

When I go to add a new post, it looks fine

When when I go to add a row, the layout breaks

Can suggestions? It’s only happening on this new website that I a building. No issues on my other projects.

field_name of Name field? => if it is name try to change it to something else.
did you have same problem? => if you change Layout of repeater to block or row

Thanks for the feedback. I tried changing the field names and got the same results. However switching it to the row layout seemed to solve the issue.
