How use ACf Text Fields based on User field

Hi,
I tried to use: https://github.com/Hube2/acf-dynamic-ajax-select-example/tree/master/dynamic-text-based-on-user-select

In ACF 5 pro I’ve imported group_57ab9d41279f5.json
In my function.php (child themes) I’ve include:

include dirname(__FILE__) . “/dynamic-text-based-on-user-select.php”;

If try to view source code ynamic-text-based-on-user-select.js is load fine…

But if I create a new post and try to select user… field: “Username, Display name and email” they remain empty.

Where am I wrong?
