how to create a sub choosen menu?

i wanna use the acf to create a sub choose menu().
like the “button group” combination “repter”

Demo:
i have 5 first level fileds,such as A, B, C, D, E
i have 25 second level fileds,such as:
a1, a2, a3, a4, a5;
b1, b2, b3, b4, b5;
c1, c2, c3, c4, c5;
d1, d2, d3, d4, d5;
e1, e2, e3, e4, e5;

my furture:
1) when i choose A, it will show a1,a2,a3,a4,a5, and i can choose either of them.
2) when i choose B, it will show b1,b2,b3,b4,b5, and i can choose either of them.
…
3) the first level: A, B, C, D, E, it’s the only one, only one of them is allowed choosed.
4) the first level or the second level must be choosed.
