Flex Content – edit image broken

Hi all, having an issue with ACF Flex Content, added an image field as per the tutorial video, all is fine at this point.

add in an image to a custom post type using flex content, image inserts fine, hover over the image and click the edit button and the edit image modal is completely blank, the form is not populated or even generated.

Am a bit stuck on this one, so any help would be great.
