Field image inputs not appearing

A couple of image fields on my taxonomy have no input controls appearing. Upon inspection the image tags are empty.

If you view the attachment, the ‘icon’ and ‘small icon’ fields are set up exactly as ‘thumbnail’ is, and while they have worked in the past and have images assigned to them, for some reason they are not showing.

If you check the source code in the inspector are there any proper image tags at all?
