Conditional Fields Do not show Data

I have a checkbox “Specific Date TBD”

I have two date picker boxes, that will show/hide depending on the status of the checkbox: “Start Date” or “Start Month”. If I check “Specific Date TBD” and enter a Start Month, the data appears to save properly, and I can see it on the front end of my site. However, if I go back to edit the post, it will properly check the “Specific Date TBD” box, and it will display the “Start Month” field (instead of the “Start Date” field), but the value for the Start Month will not be set.

Can you please include some screenshots of the issue? Or perhaps a screencast?

Just to clarify, are any of your fields using the same field name?
