Can't render a front end form.

Hi, I am trying to use ACF to create a front end form. I want a user to submit information via this form, then take that data and create/update an informational page.

I followed the brief tutorial provided by ACF, but so far all that renders is an update button at the top of the page.

I'm fairly new to WordPress, so perhaps I just put something in the wrong place? Any help would be appreciated, thanks!
