Repeater Field as a individual Form

Hello, i have a CPT with this repeater Group.

CPT
– Excercise
– Instructions
– User Note (text field)

If i create a form for “User Note”, and i place it into the loop, when the user comment in “user note”, he save all field “user note” with the same content of any “user note”.

In short, I have to add a user note field, which appears on each exercise and that the user can fill so that the administrator sees him both from Frontend and backend.
