Issues with Checkbox Label

I used the documentation on here and just copied it over and replaced the value with mine and even still I can’t reference the checkbox label.

I’m looking to do something like this with multiple choices:

<div><img src="http://www.domain.com/'checkbox_value'.png" /> 'Checkbox Label'</div> 

Can somebody assist?
