how to create the url with the fields value?

i create a custom fields named ‘visa’, and it have a value ‘usa’,
how can i create a url like “http://aaa.com/usa”
this url will show all posts with have the custom value ‘usa’

and how can i filter the post with the category and custom fields both?
eg:choose the category is AAA and the custom fields visa is usa.
i wanna create the url: http://www.aaa.com/AAA/usa
