It could be a permission issue or any other things. In this case, could you please ask your hosting about it?
Also, it seems that you’re manually adding the Google Maps script on the page. Could you please remove it and use the acf_form_head() function instead? This page should give you more idea about it: https://www.advancedcustomfields.com/resources/create-a-front-end-form/.
Also, don’t forget to add the API key by following the guide in the link I gave you before (https://www.advancedcustomfields.com/resources/google-map/#google-map%20api).
