I believe the easiest way would be something like this:
// check if the repeater field has rows of data
if( have_rows('repeater_field_name', 'option') ): $i = 0;
// loop through the rows of data
while ( have_rows('repeater_field_name', 'option') ) : the_row(); $i++; ?>
<div class="<?php if ($i == 1){echo "active ";} ?>item">
<?php
// display a sub field value
the_sub_field('sub_field_name');
?>
</div>
<?php
endwhile;
else :
// no rows found
endif;
