The problem still occurs for us. It is not on every post and we can go weeks without an issue and then it happens again, which has made it much harder to troubleshoot.
I have found that previewing a post before publishing can cause the issue. We schedule 4 posts every evening to post the following morning, if the publishing order is changed after they are scheduled by changing the post time, this seems like a possible trigger.
